Chronic disease management (CDM) is a burgeoning problem. As the population ages, the prevalence of chronic diseases is expected to rise. Compounding the problem, chronic diseases account for far more health spending than acute conditions. CDM also differs from acute care, in that the conditions are “managed” rather than cured or resolved. They require a distinctly different treatment approach.
